Senior Alternatives is a geriatric care management and home care organization that has been serving Alameda and Contra Costa County and the surrounding Bay Area since 1995. Our interdisciplinary Care Management team are Masters-level geriatric specialists who have years of experience working in areas of dementia, hospice, chronic medical conditions and disabilities. At Senior Alternatives we employ experienced, compassionate, and well-trained Caregivers, who work collaboratively with our Masters-level Care Management staff. In addition, we have developed relationships with reputable physicians, nurses and other geriatric professionals and utilize these experts as appropriate. Getting a plan of care in place and implemented can be very challenging: often the client is not open to receiving care or making changes. Using a social work agency model, our care managers guide and support the family to ensure that the client gets the help they need to thrive. All members of our interdisciplinary team are Masters-level geriatric specialists who have years of experience working in areas of dementia, hospice, chronic medical conditions, and disabilities. In addition, we have developed relationships with reputable physicians, nurses and other geriatric professionals and utilize these experts as appropriate. After an initial consultation and decision to move forward, a Care Manager will arrange a time to visit the client and involved parties at the client’s place of residence and complete an initial psycho-social assessment. Based on information gathered during the initial assessment, the Care Manager develops a care plan to address areas of concern. Recommendations may include: home care; education; symptom management for dementia and mental illness; managing family caregiver stress; alternative living arrangements; socialization plans; medical, legal and financial referrals; and benefit entitlements. In addition, the Care Manager can serve as the liaison between health care providers, social service agencies, the client, and involved family members. Care plans are regularly monitored for effectiveness and adjusted to address changing needs. We understand that selecting the right Caregiver for a family member is the first step and most important aspect of a successful, holistic care plan. As a private Care Management organization, we have the clinical expertise and knowledge necessary to match the right Caregiver to meet our client’s short and long term needs. In addition, we are adept at matching the skills and personalities of our Caregivers so that they are compatible with our client’s individual physical and emotional needs. We realize that the Caregiver is a pivotal member of any care plan team — a good Caregiver can mean the difference between a person being able to remain safely in their home, or having to move to an institutional setting. Since we have a social work centered approach, we are able to define and develop the most cost-effective caregiving schedule, as well as determine whether home care is economically feasible. Aging Peer Care (APC) is an alternative to our existing home care program. While we always aim to match our caregivers’ skills and personalities with clients’ physical and emotional needs, APC caregivers bring a different kind of compatibility. APC caregivers are over 50 years old, and have similar life interests, age and experiences as our clients. With APC, you get the help you need and so much more; it’s about adding joy and companionship back into life. When you’re with a peer, getting care really can be fun. You might do things such as crafts and games, organize your mail and other papers, plan out some meals, or go out for a walk. Your peer caregiver may also perform the usual light housekeeping, personal care, transportation, and other needed services. As with all of our services, we practice careful infectious disease control for caregivers and our clients. Service based on peer caregiver availability.

In addition to working with clients and their families, Senior Alternatives offers consulting services as well as customized programming to meet your organization’s needs. We also are able to provide in-service training to social workers, nursing staff and caregivers. We work with medical groups, employee assistance programs, housing associations, social service agencies, universities, facilities and private companies. We are available to consult on a broad spectrum of areas from client care to program development and staff education. Some examples: For the San Francisco region of one of the largest national disease social service agencies we developed and provided a care management program for the San Francisco Bay area. For a large regional medical organization and HMO we created a care management program that reduced post-hospitalization rehabilitation costs, as well as costs associated with repeat hospitalizations. For a University of California campus we performed the care management assessments for a care management pilot program.
